A psychologist is a trained mental health professional who helps people learn healthy ways to handle mental health challenges. Our practice psychologist can work with you if you suffer with depression or anxiety.  The psychologist can also support patients who have been recently discharged from psychology services and require support implementing relapse plans.

Your GP can refer you to the practice psychologist.

A health and wellness coach, is a supportive guide who can help you set health goals, whether to lose weight, stop smoking, improve energy or better manage stress.

Health and wellness coaches are vital members of the greater healthcare team, motivating clients for sustainable and positive behavioural lifestyle changes.

Our clinical pharmacists are part of the clinical team. They provide extra help to manage long-term conditions, advice for patients on multiple medicines. They carry out structured medication reviews for patients with ongoing health problems. They are here to answer any questions patients may have about medications they are on.

We offer home visits for patients who are genuinely housebound because of illness or disability. A GP or Paramedic will only visit you at home if your medical condition requires it.
Please try to telephone reception before 10:30am if you think you may require a home visit. We will take your details and ask a doctor or paramedic to phone you back to discuss the matter. Some problems can be safely managed with telephone advice, however a home visit will be arranged if clinically indicated, and the clinician will decide how urgently a visit is needed.
You can also be visited at home by a Community Nurse if you are referred by your GP. If you have recently had a baby or if you are newly registered with a GP and have a child under five years old, you can be visited by a community health visitor

Please remember that multiple patients can be seen at the practice in the time that it takes to make a single home visit. We also have better facilities on-site for examining and treating patients. Thus we fully explore all alternatives before confirming whether a visit will take place or not. We are only able to provide home visits to patients living within the Practice catchment area, SW18.
